Medical
Blue Cross Blue Shield HMO
The HMO medical plan requires you and your family members to select a participating Blue Cross Blue Shield Primary Care Physician (PCP). The PCP you and your family members select will direct you to the right specialist and will work with the specialist to guide your treatment and recovery. The HMO plan includes over 2,500 Primary Care Physicians and 3,000 Specialists at over 85 Chicago-area hospitals.
Blue Cross Blue Shield PPO
The PPO medical plan allows you and your family members to see any physician within the Blue Cross Blue Shield PPO network. The PPO network includes over 20,000 physicians in the Chicago area. This plan offers 80% coverage for covered, in-network services after applicable deductibles are met.
Prescription
Retail Pharmacy
Enrollment in the Blue Cross Blue Shield medical plans will automatically enroll you in the Blue Cross Blue Shield prescription plan. The BCBS prescription plan offers coverage for generic, formulary and non-formulary drugs.
Mail Order Pharmacy
Mail Order Pharmacy benefits are available with both the HMO and PPO Blue Cross Blue Shield medical plans. The mail order program is a convenient option for ordering medications you take on a regular basis. The mail order pharmacy can save you time and a significant amount of money. Under the mail order benefit, you can receive a 90-day supply of your prescription for the cost a 30-day supply at the retail pharmacy.
Vision
Blue Cross Blue Shield HMO Vision Care
As an HMO Illinois member, vision benefits include an annual routine vision examination for you and your eligible dependents for the same cost as your office visit co-payment (a referral from your primary care physician is not necessary). In addition to your vision benefits, you will receive a vision discount on the purchase of eyewear. EyeMed doctors offer a 20% discount on materials and will deduct additional benefits from the amount of your eyewear purchase once every 24 months. Your vision discount program also includes a discount on laser vision surgery!
Blue Cross Blue Shield PPO Vision Care
Blue Cross and Blue Shield of Illinois PPO members are automatically enrolled in a vision discount program administered through EyeMed Vision Care. EyeMed Vision Care provides access to more than 18,000 vision care providers nationwide — and close to 1,000 providers in Illinois.
Under the Blue Cross and Blue Shield PPO medical plan, you and your eligible dependents can enjoy access to more than 18,000 eye care professionals nationwide including private practice opthamologists, optometrists and opticians. Discounts on eye exams, and savings of up to 45% on the purchase of eyewear, including lenses, frames and contact lenses.
Dental
First Commonwealth Dental HMO
The First Commonwealth Dental HMO plan offers the largest Dental HMO network in Illinois. The Dental HMO plan offers an opportunity to have greater coverage and pay less out-of-pocket costs for dental care by receiving care from a First Commonwealth Participating Dentist. Each family member may select a different participating dentist. All covered services are provided by the general dentist you select. Payment is made at the time of service, and all participating dentists have agreed to a discounted fee schedule.
Guardian Dental PPO
The Guardian PPO dental plan allows you unrestricted provider choice. You can receive dental care from any dentist, within or outside of the Preferred PPO network. Guardian offers a Preferred PPO network, which is one of largest dental networks in Chicago. Participating PPO dental providers agree to discount their fees; therefore, using a PPO dentist can lower your out of pocket expenses.
Galter Lifecenter Memberships
Galter LifeCenter memberships are available to all employees at a discounted rate, with no annual contract and no initiation fee. The beautifully renovated facility has a swimming pool, running track, exercise equipment and aerobic and yoga programs. The LifeCenter also offers a variety of athletic, medical and wellness programs and even provides on-site child care services.

Swedish Covenant Hospital Employer Discounts
Employees and their dependents are encouraged to use the many available hospital services. Employees who have insurance coverage through Swedish Covenant Hospital will have the co-insurance waived after paying the deductible and applicable co-payments. Employees who have other insurance coverage will receive a 50% discount to the remainder of the bill after the insurance carrier has processed the claim, and any applicable deductibles have been paid.
Employee Health Services
As the point of entry into the LifeWORKS wellness program, Employee Health Services offers a variety of vaccines/flu shots, blood pressure readings and health risk appraisals. Employee Health Services manages employee safety issues, ergonomics, back safety, hazard communication and personal protection.
